Possibilities:

The beer glass was "chilled" in an ice bin before pouring the beer. Somehow broken glass got into the ice and adhered through suction to your boyfriend's glass.

Your boyfriend's beer glass was washed in a commercial dishwashing unit which is overloaded and a glass breaks. When handled, the suction from the water allows a bit of the broken glass to adhere to your boyfriend's glass even after it was put away upside down

The bartender mixes several drinks at once, with glasses lined up at the well. When scooping ice into other beverages a bit of glass falls into your boyfriend's beer because there was broken glass in the ice.

The beer glass was chilled in a refrigerator where the glasses were stacked too closely together and one broke. When your boyfriend's beer glass was used to pour the beer, there was a bit of glass inside it....

These things happen on occasion. The manager probably should have conveyed more concern, but buying your drinks was an appropriate response.Thankfully, these instances are quite rare...
