You see it all the time and sometimes I want to smack people.
The problem is, it's up to the clerk to be nice and friendly and polite to *all* customers no matter how hideous they are -- and it's up to the company to make sure it happens. So many just don't seem to care. It's amazing they stay in business.
It cracks me up even thinking about what happened in Taco Bell one night:
A lady got her burrito and it was wrong. She very politely asked if they would fix it. The clerk told her she ordered it that way. She politely said she did not, and she would like it fixed please; she couldn't eat it as it was. The clerk said she read back her order and the woman didn't say anything. The woman said (still amazingly nicely) that she didn't care what she said, she would even pay for it, but please change it so she could eat.
The clerk CALLED THE MANAGER (understand the customer was not being beligerent or rude or even testy).
The manager listened to the story and ... get this ... proceeded to politely TELL THE CUSTOMER why it was her fault! I remember it like it was yesterday: "But it was your fault, ma'am, that's why she's trying to explain. See, once she repeated the order back to you, then it became your responsibility. It became your fault."
We're lookin' at a 79-cent burrito here.
Oh BWAHAHAHAHAHAHAHAHAHAHAHA!!
